Edit The Add Lead Form
This article will guide you through modifying the Add Lead Form. The Add Lead form is found by going to Leads/Prospects > Add A New Prospect. You can customize this form to capture exactly the information you want when adding a new lead. 

 

RequirementsA Master Admin login or a staff type with the following permissions:

  • Club Setup: Can access setup 
  • Club Setup: Can access add lead form setup


Add Lead Form Setup




To access this screen click on Setup > Leads > Add Lead Form. The green column on the left can be selected so you can add or remove fields from the form.  The orange column on the right is how you can toggle Yes/No of whether a field is required.




The Email and Cell Phone fields are always included on the form but you may choose to also include Home or Work Phone. If you do not have a preference as to which phone number the customer provides you may choose At Least One Phone Number Entered and make this required. This will not limit which phone number field is required, as long as one field has been completed.





Lead Type - Assigning the customer a Prospect/Lead Type which can be used to organize prospects through Filters or release Automated Prospect/Lead Rules.                                                            

Referral Type - The marketing method that made this customer decide to contact your club. The Referral Types/Marketing Campaigns are options your may customize for your club.

Contacted Club How - How this customer reached out to you.

Initial Contact With - The employee the customer initially had contact with.

Assign Lead To - Allows the lead to be assigned to a specific employee.




Assign Free Booking Credits - The ability to assign the customer free consultation credits when adding them as a new Prospect/Lead.

If No Free Booking Credits-Determine Why - Will provide a drop down of reasons to choose why free credits where not given to the customer. Reasons such as Time Constraints, Cost, Not Interested, and Other.

Make Login Available - Choose if you would like to provide the lead a login to the Client Wellness Portal. You can choose anywhere from no access, 1 - 4 weeks access, or unlimited access.

Key Info Note Entry - Add a note that appears on the first Info page after you select a customer name. 

Assign Check In Barcode or Pincode - Offers the opportunity to assign their check in information.

Assign Primary Provider - Assigning them to a primary Trainer.





Date of Birth and Address will always be available on the form but you may determine if you would like them to be required.

Drivers License - A field to enter the Lead's license number.

Current Weight & Height - Add this data to their profile for reference when setting goals.

Main Fitness Interests - This data is stored on their profile to view the Lead's interest in being a part of your gym.

Emergency Contact and Doctor Contact Info - Data can be added and stored on the Lead's profile.

Exact Duplicate Entry Check-Don't Allow If Duplicate Name & Phone - Prevents creating duplicate profiles.




There is also an option to include any Custom Categories or Custom Text Fields to the form and make them required if desired.  You can also create a new custom fitness goal.



To save your changes to the prospect form click the button Save Changes To Your Add Lead Form Setup.
