Membership is a 12-month agreement that renews on a month-to-month basis after. After 12 months, monthly dues increase to $14.99. Annual fee of $49.99+ tax billed 90 days after joining. To cancel within the first 12-months you must do so in person or via certified mail and provide a doctor's note or proof of move 25 miles or more away. After 12-months, you can cancel at any time for any reason. When you cancel, you are responsible for one additional month during which you can still use the services included. If your annual fee falls in a month when your membership is still active, you are responsible for this as well. Cancellations are never accepted via phone, fax, or email.
