The Member hereby purchases a membership in the health fitness club facility operated by ANATOMY at 3415 NE 2nd Avenue Miami, FL 33137 for a period of time beginning today. Membership will continue on a month to month basis until terminated by the member. I understand that monthly dues will be billed on the 25th of each month and shall be billed to the credit card used for payment today. BY AGREEING TO THE TERMS BELOW, THE MEMBER WARRANTS THAT HE/SHE HAS READ, UNDERSTANDS, AND AGREES TO ALL CONDITIONS OF THIS MEMBERSHIP AGREEMENT.

This membership may be cancelled at any time with 30 day written notice by the Member to Anatomy via email memberservices@anatomyfitness.com, at which time final Monthly Membership Dues will be deducted from the bank account of the Member or billed to the credit card account of the Member. All cancellation requests are accepted via email to memberservices@anatomyfitness.com.
