The Member hereby purchases a membership in the health fitness club facility operated by Anatomy located at 1220 20th St Miami Beach, FL 33139 with access to Anatomy Miami Beach for a period of time beginning today, and continuing for 12 months thereafter. After the term, membership will continue on a month to month basis until terminated by the member. I understand that monthly dues will be billed on the 25th of each month and shall be billed to the credit card used for payment today.
In order to finalize your Membership and gain access to your club, we ask that you stop by the front desk upon your first visit where Management or your Sales Liaison would be happy to assist you!
BY AGREEING TO THE TERMS BELOW, THE MEMBER WARRANTS THAT HE/SHE HAS READ, UNDERSTANDS, AND AGREES TO ALL CONDITIONS OF THIS MEMBERSHIP AGREEMENT.
How To Cancel
Within the 12 month term, this membership may be cancelled with proof of relocation of a minimum 25 miles from club or medical note from a doctor and will require one (1) month of membership dues collected at time of cancellation. After the 12 month term, the membership may be cancelled at any time with 30 day written notice by the Member to Anatomy, at which time final Monthly Membership Dues will be deducted from the bank account of the Member or billed to the credit card account of the Member. All cancellation requests are accepted via email to firstname.lastname@example.org.
