The Member hereby purchases a membership in the health fitness club facility operated by Anatomy located at 19505 Biscayne Blvd., Suite 3150, Aventura, FL 33180. Your commitment period begins at the time your selected club opens. I understand that monthly dues will be billed on the 25th of each month and shall be billed to the credit card used for payment today once the club opens. HOME CLUB: Your “Home Club” is the Club at which your Membership was purchased and is listed at the beginning of this Agreement. If your Home Club is not yet open when this Agreement is executed, Member may elect to use the services a different Club during the presale period subject to managements approval and certain conditions. -- BY AGREEING TO THE TERMS BELOW, THE MEMBER WARRANTS THAT HE/SHE HAS READ, UNDERSTANDS, AND AGREES TO ALL CONDITIONS OF THIS MEMBERSHIP AGREEMENT.

If the Member has purchased a membership prior to the club opening, the Member may cancel the agreement at any time before the club’s opening. Once the club has opened, member may cancel this agreement, but must do so within seven (7) days of the date that the club opens for business, excluding weekends and holidays. To cancel this agreement, you must provide written notice, which states that you, the member, are cancelling this agreement, or words of similar effect. You may provide this notice by way of e-mail sent to memberservices@anatomyfitness.com. Within the 12 month term, this membership may only be cancelled with proof of relocation of a minimum 25 miles from club or medical note from a doctor and will require one (1) month of membership dues collected at time of cancellation. After the 12 month term, the membership may be cancelled at any time with 30 day written notice by the Member to Anatomy, at which time final Monthly Membership Dues will be deducted from the bank account of the Member or billed to the credit card account of the Member.
